Initial Appointment

If you are coming to the Center for the first time, please make sure that you bring with you the following information:

• Your insurance card
• Photo identification
• A list of your medications and allergies (if any)
• Information about your family history of cancer, and breast and ovarian cancer specifically.

If you are coming for a biopsy, be sure that you have stopped all anticoagulants such as Aspirin or Plavix at least 1 week before your appointment.  If you are taking Coumadin, you should check with your doctor about discontinuing your medication before the biopsy.

If you have Adobe Acrobat Reader, you can download and print the registrations forms, print the registration forms (click on link on the top of the page) so you can complete them prior to your first visit. If you don't have Adobe Reader, you can download if for free from the link above. 

If you cannot complete the registration forms before your first visit, please allow at least 15 minutes before your appointment to complete the forms.
